HOW TO VOLUNTEER ON CAMPUS

Before a parent may volunteer in our classrooms, work an on-campus event during the school day, chaperone a field trip and/or coach a school sports team,  you must have all of the below on file with School Office. Please send requested documentation to Mrs. Caroline Cota, ccota@olfdof.org. 

• Obtain LiveScan Fingerprint Clearance. In Los Banos, you may make a LiveScan appointment at the Police Station or UPS Store. Before your appointment, pick up the Fresno Diocese LiveScan form from the School Office. 
• Complete Safe Environment Training, available in English or Spanish on the Fresno Diocese Website. Safe Environment certification must be renewed every 5 years. Click HERE for instructions on how to complete the training. Click HERE to jump to the Fresno Diocese training login. 
• Review and sign the Fresno Diocese Code of Conduct. This must be completed every school year. Click HERE to print a copy. 
• Complete Tuberculosis (TB) Risk Assessment. Email School Nurse Nicole Gallichio at ngallichio@olfdof.org to schedule a TB Assessment.

To drive your own child for a field trip, sports or other school-sponsored event, you must meet all of the requirements on the Parent Volunteer Checklist, above. 

To drive any other OLF students for school-sponsored events, you must meet all of the requirements on the Parent Volunteer Checklist AND submit copies of the following to Mrs. Caroline Cota, ccota@olfdof.org:

• Driver's License (must resubmit when license expires)
• Vehicle Registration (must resubmit annually)
• Automobile Insurance Declaration Page with policy limits (must resubmit annually)
